Houghton care also offers a hospital-to-home, rapid response service allowing faster discharges from the hospitals. We accept referrals both during office hours and out of hours.
We provide:
• Personal care, shopping, light domestic chores, meal preparation and feeding, companionship, escort duties, support with medication, palliative Care and live-in care.
• Rapid response services, to enable faster discharges from hospitals to home
• We operate a discharge-to-assess scheme offering continued monitoring of the patient's care needs.
•Houghton Care works in collaboration with other professionals to create a tailored, person-centred care plan for each individual.
• Houghton care offers live-in care services as well as home care support, whether short or long-term, with care support available in as little as 24 hours.
