The care and support services provided by our organization are defined under the Standard care Contract which is formulated through comprehensive assessment of needs of each individual service user. These services are mainly split into three groups:
• Essential
• Additional
• Enhanced
Essential services
We provide essential services for older people who have various health conditions which include dementia. We work collaboratively with their medical practitioners, consultants and other professionals to manage other complex physical health needs.
Our core services include:
• Personal care
• Support with activities of daily living
• Support with medication administration (blister packed only)
• Support with liaising with other agencies
Additional Services
• Training provision
Enhanced Service
• Assessing for regulatory requirements
• Setting up other services
