Supported Living and Community Support
HFHC Healthcare provides carers and support workers to support individuals who live in their own home, including single and multiple occupancy supported living communities.
Support includes personal care and physical support, mental health support and companionship for people needing a little extra help – from a few hours a week to 24-hour care.
Our specialist services include care for older people as well as supporting people with physical and mental health conditions, autism and learning disabilities. This covers a wide range of conditions including dementia, Alzheimer’s, epilepsy, anxiety, bipolar disorder and schizophrenia.
Bespoke care packages are carefully built around each individual to offer the most effective and respectful support, enabling them to live as independently as possible.
